Patient-Centric Healthcare Requires a Brand-New Approach to Personalized Engagement

Let's address the elephant in the room rather than continuing to ignore it: traditional patient and member engagement methods are no longer sufficient. Despite their once-perceived actionable impact, postcards, emails, and notification-driven smartphone-based apps fail to deliver meaningful outcomes for patients and members.

It's clear that we need to embrace a member- and patient-centric approach—one that prioritizes personalized engagement to improve clinical outcomes while reducing healthcare and operational costs.

Challenging the status quo: rethink traditional engagement

The limitations of conventional engagement methods are glaring. Beyond the exorbitant costs of postcards and smartphone apps, these approaches are ineffective.

Whether it's about engaging high-utilizers or reaching Medicare populations, the absence of meaningfully designed engagement is palpable. It also reduces satisfaction and care quality while continuing to contribute to high healthcare costs for all stakeholders.

We must improve health outcomes and lower healthcare costs while reducing operational spending so that all stakeholders can benefit. To achieve this, let’s boldly re-imagine our approach to patient and member engagement.

Embracing innovative healthcare communications: Meet members and patients where they are.

Imagine having an unlimited number of call center employees reaching every member or patient for several different personalized reasons at a fraction of the cost. Imagine boosting engagement among members and patients by 8 to 12 times at a fraction of the cost. Finally, imagine opening up capacity for your nurses and care managers by a minimum of 600%.

Imagine!

Here’s the reality: a lot of healthcare happens at home, especially for older adults who often deal with significant health issues. So, connecting with them where they're most comfortable only makes sense.

But instead of inundating people with generic communications, let's tailor our engagements to their individual needs and contexts and in the languages they speak. By adopting strategies that include proactive, outbound check-ins, we can establish deeper connections with members and patients by empowering them to navigate their health journey with confidence and autonomy.

The challenge: healthcare spending goes up while quality, satisfaction, and outcomes go down

Imagine a 70-year-old man struggling with multiple comorbidities, food insecurity, and the inability to afford his medications. With conversational AI, we can promptly identify gaps and connect him with community resources and support services. Informed by guided conversation that produces rich data, this personalized support is available to Medicare Advantage, Medicaid, and Commercial populations.

What results should we expect? Care gaps getting addressed immediately, the delivery of preventive care, and improved wellness over time. We can now reduce this older adult’s risk of health complications. And ultimately, we can lower overall healthcare costs when this approach is used across populations of patients and members.

Transforming chronic care: technology is the driver

Similarly, members and patients managing chronic conditions like diabetes who have access to Medicare Advantage, Medicaid, or Commercial health insurance will benefit from faster access to precise and tailored resources.

With conversational AI, we can collect and process large amounts of data quickly to provide personalized communication via multiple channels—be it text, phone, or chat. From increasing benefit awareness and enrollment to supporting chronic condition management and everything in between, personalized engagement empowers members and patients to make informed decisions, leading to improved health outcomes—at demonstrably lower costs.

A call to action: embrace the future

It's 2024. We must wholeheartedly embrace this member- and patient-centric mindset. Let's move on from outdated methods and invest in solutions that speed up resolution time for members and patients, saving everyone time and money. By prioritizing regular outreach and connections, we can make personalization and automation the new standard.

What I love about EmpowerHealth is our commitment to identifying, educating, activating, and continuously managing and monitoring individuals' health journeys. With our innovative AI-enabled digital health assistant, informed by rich data and clinical feedback, we ensure that each member or patient has an experience that's intuitive, trusted, reliable, scalable—and above all, personal.

It might sound complex, but we're simply checking in on members and patients to help them achieve wellness.

Embrace a shift toward personalized care.

Let's embrace the use of data to create a better experience for members and patients—and a rapid shift towards personalized, proactive engagement. Together, we can shape a healthcare landscape where every member or patient gets the tailored support they deserve—empowering them to lead healthier, happier lives—and reducing healthcare costs for all of us.

What does it mean to empower members and patients?

Empowering members and patients means helping them feel like they have a real say in their health. It's about believing they can tackle their health problems and control their health conditions and the life circumstances that affect their well-being.

The Data Revolution: Analytics Expertise Takes Center Stage in Managed Medicaid

The spotlight is on population health capabilities among health insurers. For proof, look no further than recent Requests for Proposals (RFPs) from state Medicaid programs that emphasize two key areas: health equity and social determinants of health. That’s according to reporting by RADAR on Medicare Advantage.

Six states, impacting approximately 11 million lives, have signaled a shift in managed care by adding new populations. Specifically, states are urging the integration of behavioral health, Managed Long-Term Services and Supports (MLTSS), and pharmacy services into acute care.

For instance, Georgia is set to transition its older, blind, and differently abled Medicaid population to managed care, while Virginia aims to consolidate MLTSS and managed Medicaid plans. These changes underscore the imperative for insurers to adapt to evolving Medicaid needs.

Analytics expertise in demand

State Medicaid agencies are seeking improved capabilities among health insurers. The focus is on insurers equipped with tools to track outcomes and streamline processes, which recognizes data's crucial role in enhancing managed Medicaid efficiency.

Centene Corp.'s CEO, Sarah London, highlighted this trend, telling RADAR on Medicare Advantage about the significance of Georgia's RFP, which includes the state's aged, blind, and differently abled population. The evolving managed Medicaid landscape presents challenges and opportunities for payer executives. Increased RFP activity and new population inclusions call for a strategic approach among health insurers.

To thrive, payers must prioritize innovation, invest in advanced analytics, and align services with evolving Medicaid needs.

Enter EmpowerHealth. Payers across the nation rely on our platform to keep members on track with their care plans and bridge crucial care gaps. Our promise? Elevate quality, drive improvements in Stars and HEDIS measures, and ensure members achieve health and well-being.

Why EmpowerHealth? One major national payer client slashed a whopping $62 million in clinical and operational costs over just 18 months.

We're revolutionizing engagement and simplifying navigation, and boosting member education—all while helping to reduce the cost of care.

Medicaid Directors Expect Medicaid Enrollment to Drop by Almost 9%

A sharp drop in Medicaid enrollment–of 8.6%–is expected in 2024, according to reporting by The Hill. Of course, the larger context is the Medicaid unwinding that started with the end of the public health emergency in May.

The drop in Medicaid enrollment is just one pain point revealed in a recent KFF survey of the nation’s Medicaid directors.

Here are 4 more learnings about the Medicaid unwinding from the KFF survey:

  1. 1. More than 1 million low-income people no longer have access to Medicaid, as of November 8.

  1. 2. At least 2 million children (likely an undercount) have lost Medicaid coverage.

  1. 3. More than 70% of people lost Medicaid coverage due to procedural reasons, such as state officials’ mistakes or lost paperwork.

  1. 4. State spending on Medicaid is expected to increase by 17% next year as a result of the phasing out of federal financial assistance.

While Medicaid directors attempt to contend with these and other realities, they’re also faced with workforce challenges. In a recent KFF webinar, Amir Bassiri, deputy commissioner of the Office of Health Insurance Programs and New York State Medicaid director, and Jennifer Strohecker, Medicaid director, Utah Department of Health and Human Services, pointed to labor shortages as a top issue they’re facing and expect to contend with in 2024.

To help address these and other challenges, EmpowerHealth supports Medicaid managed care organizations (MCOs) in achieving ambitious targets by:

  • Addressing labor shortages and economic impact

  • Rapidly identifying members and patients with outdated contact information

  • Confirming mailing address and other contact details for members and patients

  • Driving SDOH efforts

  • Educating members and patients on how and why to re-enroll

  • Sending text messages with education and other resources

  • Improving program enrollment efficiency and outcomes

MCOs around the country rely on Angel, an AI-assisted engagement service developed by EmpowerHealth that is purpose built to drive improvements in health and well-being. Some of the nation’s largest health organizations partner with EmpowerHealth to boost communications, help close care gaps, mitigate risks, and improve clinical and operational outcomes.

One of our large national payer client’s netted $62 million in clinical and operational savings over 18 months.

We’re optimizing engagement, navigation, and education for our clients, while reducing the cost of care. By identifying and addressing care gaps in real time, our clients capture care opportunities and boost quality, while improving intervention for members and patients who need it.

How Personalized Communications Help Patients on Their Health Journey and Reduce Hospital Readmissions

3.8 million. That’s the number of 30-day all-cause hospital readmissions among adults each year, according to AHRQ.

Heart failure is among the top five conditions for initial admissions. This condition also has the highest number and highest rate of readmissions for both Medicaid and self-pay/no-charge stays, per AHRQ.

Here are four more facts you need to know about hospital readmissions:

  1. 1. Twenty percent of hospital readmissions among adult patients are associated with four conditions: sepsis, heart failure, diabetes, and chronic pulmonary disease (COPD).

  2. 2. The overall cost of avoidable hospital readmissions is between $15 billion and $20 billion annually. The average cost of a single readmission for a patient on Medicare is $15,000. This harms health providers and payers where it hurts: cost, quality, and clinical outcomes. It’s also stressful for patients and members who would prefer to be on the journey to wellness.

  3. 3. The average readmission rate is 14%.

  4. 4. Nearly 17% of readmissions are covered by Medicare. That’s the highest rate of readmissions across payer types.

The Institute for Healthcare Improvement points to three ways to reduce avoidable readmissions. These include:

  1. 1. Improve core discharge planning and transition processes related to hospital stays.

  2. 2. Improve transitions and care coordination between care settings.

  3. 3. Enhance coaching, education, and support for patient self-management.

To reduce hospital readmissions, health systems and health insurers make outbound calls to patients and members when they’re discharged from the hospital. However, care managers are already fielding 50 to 100 calls per day.

EmpowerHealth can help. Health insurers, hospitals, and practices partner with EmpowerHealth to cover the same key points with patients as a live agent, which makes your contact center agents more productive.

Every single day, our clients rely on EmpowerHealth to deliver multi-modal conversation and augmented intelligence to support the health and well-being of their patients and members.

EmpowerHealth is proven: One of our large national payer client’s netted $62 million in clinical and operational savings over 18 months.

We’re optimizing engagement, navigation, and education for our clients, while reducing the cost of care. By identifying and addressing care gaps in real time, our clients capture care opportunities and boost quality, while improving intervention for members and patients who need it.

CEO Corner with Bud Flagstad: Helping Payers During the Medicaid Redetermination Process

To reach Medicaid members, payers are looking for a solution that will meet members where they are, resulting in effective engagement at the lowest cost.

Technology that boosts efficiency helps increase engagement and capture qualifying information, such as employment, income, and health and risk status. Payers also need help navigating members to take the appropriate action to retain their Medicaid coverage.

Share this news:   LinkedIn   |   Twitter   |   Facebook

Care Angel’s simple and effective approach helps payers. Our 28% to 83% engagement rate with Angel is consistent across voice, secure chat, and text options. Payer clients should expect to increase their traditional outreach capacity by 600x or more with Angel, our digital health engagement platform.

We sat down for a Q&A with Bud Flagstad, Care Angel’s CEO, to discuss ways to boost outreach efforts during Medicaid determination.

Care Angel: What’s different about Care Angel?

Bud Flagstad: For starters, we have trained Angel for over seven years with medical and health language knowledge. That gives us context for conversations with members. It also allows us to automate repetitive, high-volume tasks. Our campaigns help us reach members; in this case, letting them know about the need to reapply for insurance coverage.

We also help payers identify care gaps quickly and efficiently. Angel can understand conversations in more than 72 languages. That helps us reach diverse populations across the country—how and when they want to be engaged.

Care Angel: What have you learned about when and how to contact members?

Flagstad: We have access to a wealth of data about members, which means we know when they’re more likely to respond and what type of outreach is more likely to generate responses. For example, Medicare members are less likely to respond during “The Price is Right,” while Medicaid members are more likely to interact at certain times using their smart phones via voice or text.

We can circle back with these members using voice, text, and secure chat. Unlike a traditional contact center employee, who may only have time to dial once, our technology can redial, which means we have a better chance of connecting with members. Our technology gives us the flexibility to start with voice and shift to other modalities based on user preferences.

Care Angel builds profiles of members, using the cognitive abilities of our AI model. While most plans have consent to text approximately 5% of their Medicaid population, we’re able to reach and gain consent for texting between 24% to 32% of their population, depending on the campaign. While complying with all appropriate regulations, Angel can engage with members using voice, text, and secure chat whenever they’re most likely to be available.

Staffing nurses at a call center after business hours is virtually impossible, but Angel provides the ability to engage with members—and that means we’re connecting when members are available.

Care Angel: But a payer executive may already have the budgeted dollars to staff a call center. What then?

Flagstad: Payers will save a significant amount of budget dollars by working with Care Angel—and those dollars can be used for other initiatives.

Angel captures information while providing education and navigation support for members—without adding to labor costs. Adaptable and customizable for other costly and critical engagement needs, Angel supports the following programs:

  • Enrolment and awareness

  • Maternal health and well-child

  • Diabetes quality measures

  • Chronic condition management

  • Medication and care plan compliance

  • Post-discharge/readmission avoidance

  • SDoH and health equity, and more

Care Angel keeps it simple. We're going to help you meet or exceed your Medicaid redetermination targets and keep your members covered.

5 ways AI is redesigning healthcare for the better

It’s spring and we are excited about the future of the most meaningful kind of digital health yet! Virtual care has paved the way towards using technology to deliver healthcare at a distance and member adoption is soaring. In fact, according to an article in the National Law Review, at least 7 million patients in the U. S. will access telemedicine services in 2018.

Share this news:   LinkedIn   |   Twitter   |   Facebook

Care Angel is combining the capabilities of virtual health and artificial intelligence (AI) to expand the versatility of patient engagement to create a truly patient-centered approach to care, beyond acute episodes. Patient monitoring and health management using Angel, our intelligent, virtual nurse assistant, allows doctors, payers and their patient members to communicate bi-directionally between office visits or after hospitalization to improve care plan adherence, patient satisfaction and health outcomes.

AI is actually humanizing modern healthcare and enabling more frequent connections to effectively and efficiently close gaps in care, empowering the patient at home.

Here are 5 ways that AI is helping to redesign healthcare for
the better:

1. PATIENT CENTERED

AI and telehealth are the perfect combination for personalized health monitoring and data gathering of your patient's vitals and wellbeing to foresee potential problems before they occur. Patients are becoming more tech-savvy and want to manage their health using technology.

2. CARE PLAN AND MEDICATION ADHERENCE

Comprehensive care that proactively manages chronic conditions automatically. Keeps medication adherence and care coordination in check for tens of thousands of patients. Custom workflows, including live transfers to provider helps to drive the highest quality and efficiency at the lowest cost for all stakeholders.

3. CLINICALLY EFFICIENT

Imagine if each nurse or care manager could be 300% more efficient. AI and voice solutions allow physicians and clinicians to focus on where they create the most value; the patient encounter. Angel takes the repetitive, redundant tasks away from the caregivers, so they can practice at the top of their license.

4. REDUCE MISSED CARE OPPORTUNITIES

Meeting patients “at their need”, helps the patient to stay on course with treatment plans and increases provider awareness of potential gaps in care. When a medical need arises, the provider is notified in real time with the knowledge they need, leading to improved patient outcomes and better manage whole populations.

Here’s a short video that how AI and voice drives meaningful engagement:

{{ script_embed('wistia', '140wwfqni3', '', 'inline,responsive') }}

5. COST EFFECTIVE DISEASE MANAGEMENT AT SCALE

How many times have you uttered these 5 words? “If only we’d known sooner.” Knowing in advance when a patient didn’t sleep well, was in pain, had out of range vitals, was in need of medical supplies or medical services can help protect the patient from an adverse episode or hospitalization.

How do we make healthcare better and more human?

Think of it this way: The more specifically you can identify the patients that are in need and what those needs are, the more likely it is that you’ll be able to close those critical gaps in care.

Consider what would happen if you could stop spending hours and hours trying to identify patients in need of intervention? Imagine if a nurse assistant could notify you when a patient was in need of attention. In just 1-2 minutes, care managers can review the specific alert, which includes longitudinal data, and appropriately take decisive action to resolve the issue.

The game truly changes when an entire community is rallied toward solving the challenge of making healthcare more human.

Commercial or Medicare ACOs Carrying Risk for Populations Will Reap the Benefits of Care Angel

The premise of Accountable Care Organizations (ACO) is for different entities to jointly commit to the health of the patients in order to qualify for financial incentives by achieving quality and costs goals. These incentives have stimulated more patient engagement, monitoring, and preventive care in order to reduce emergency room visits and readmissions that drive up costs. According to 2017 CMS performance data, 60% of ACOs saved money in 2017 and 34% of ACOs earned shared savings.

At Care Angel, we believe that to successfully manage the health of these Medicare populations, ACOs must monitor at a more granular level to proactively triage and treat patients in ways that will result in better health outcomes and greater shared savings.

How Does Care Angel Support ACO Partners?

Effective ACOs must share the same level of commitment to collaborating and sharing information in order to bridge gaps within patient care. Each provider must work to seamlessly communicate with patients’ and other providers to ensure that the quality of care is optimal, in order to meet the requirements for financial rewards, bonuses, and other shared savings. ACOs must also successfully rely on the data provided through digital check-ins that monitor vitals, wellbeing, and medication adherence to ultimately accomplish the goals of the ACO to reduce unnecessary emergency department visits and improve clinical outcomes.

Simply put, Care Angel removes barriers that limit timely access. Angel supports the necessary collaborative mindset that is tantamount to population health and provides real-time data and analytics that can be assessed by all stakeholders, including the patients and their family caregivers.

Why Choose Care Angel for your Innovative Population Health Solution?

A partnership with Care Angel guarantees a reduction in inefficiency and waste, and addresses a whole host of other common challenges faced by health systems, providers, and insurers, like:

  • Inefficient use of clinician time and skills

  • Ineffective method of identifying risk

  • High administrative cost and operational complexity

  • Decreased care team efficiency and effectiveness

  • Poor outcomes, resulting in readmission

It’s estimated that more than 1/3 of the nation’s $3.2 trillion healthcare spending is wasteful. Care Angel helps to eliminate waste as well as:

  • Increase provider and care team capacity

  • Identify patients who need care in real time

  • Notify in real-time and enables live transfers

  • Maximize clinical efficiency and effectiveness

  • Lower costs and improves productivity

Most Efficient and Effective: Virtual Nurse Assistant

Care Angel stands apart from competitors by using the most natural user interface, voice. Angel becomes easily integrated into user’s life, like a family member. Angel helps to continuously manage care, checking in on individuals about vitals, pain, mood, sleep, nutrition and more - and she detects, based on responses, when a member needs additional resources like the doctor, nurse helpline, pharmacy, family caregiver or other community resource. Paired with a strong commitment to clinical quality and user design, these care calls are dynamic and personal based on the needs of the member and provide real-time alerts and notifications to designated care teams and family members. The information and data that's collected is stored in a secure, HIPAA-compliant dashboard. Angel helps connect members to appropriate resources, services, educational material or family care circle, reducing the chance of an acute episode, hospital admission, or readmission.

In the world of value-based care, it is the quality, efficiency and effectiveness of care that helps to improve outcomes and member satisfaction. While other health engagement competitors offer a chat-bot, via text, only Care Angel deploys a multi-modal approach, starting with a simple phone call. The ubiquity of the telephone can effectively communicate with 100% of populations, including seniors who are often the hardest-to-reach demographic. In many cases, patients 65+ don’t have app or smartphone capabilities, and if they do, are hesitant to use them due to lack of familiarity. Our virtual nurse assistant extends digital healthcare to everyone, requires no app download, and easily relays information directly from the patient to care teams and family. This allows continuous, proactive engagement with members that creates a longitudinal record across entire populations.

Close Gaps in Care: Outstanding Population Health Model

Our technology transforms clinical and SDoH patient data into real-time actionable interventions, resulting in lower costs, more revenue generating opportunities, improved STARs and HEDIS performance and higher patient satisfaction for payers and providers.

We help bridge gaps in care. We drive proven financial savings for health plans, mitigating risk for the costliest populations. We enhance the consumer experience for digital therapeutics pharmacy, pharma, clinical trials, PERs and other partners, supporting patient needs for communication and education around care. And we give family caregivers peace of mind with notifications about their loved-one via integrated mobile and web applications that securely organize patient responses into data to provide regular summaries of health status and trigger alerts if needs or risk is found.
